Enhancing Fiscal Support and Centre–State Coordination in Public Health

A Parliamentary Committee has recommended measures to strengthen cooperative federalism in the health sector, emphasizing the need for enhanced fiscal transfers and improved coordination between the Centre and States. The recommendations come in the context of strengthening public health preparedness, disease surveillance, and healthcare infrastructure across the country.
The Committee observed that public health is primarily a State subject under the Constitution, but national health emergencies and large-scale disease outbreaks require coordinated action and financial support from the Union government. It recommended increasing untied and flexible fiscal transfers to States, enabling them to address region-specific health challenges while aligning with national priorities.

To improve preparedness, the Panel suggested establishing institutionalized coordination mechanisms, including regular Centre–State consultations, integrated disease surveillance systems, and standardized response protocols. Strengthening data-sharing frameworks and digital health platforms was also highlighted as essential for timely interventions.
The Committee further recommended enhanced investment in primary healthcare, public health laboratories, and human resources, particularly in underserved and rural areas. It emphasized capacity-building initiatives, joint training programs, and sharing of best practices to ensure uniform standards of healthcare delivery nationwide.
The recommendations align with broader initiatives such as the National Health Mission and Ayushman Bharat, which require effective Centre–State collaboration for successful implementation.
